Fallon Health Weinberg is a partnership between Fallon Health of Massachusetts and Weinberg Campus of Erie County, New York. Fallon Health Weinberg offers a Program of All-Inclusive Care for the Elderly (PACE) and a Managed Long Term Care (MLTC) plan to serve the health needs of dual-eligible residents of the Western New York counties of Erie and Niagara. The primary responsibility of the Outreach Coordinator is to maximize marketing efforts to generate referrals in order to meet enrollment goals that assists in the growth and retention of membership.
In consultation with the Program Director, sets referral and enrollment targets and meets them.
Assists in development of the annual marketing plan
Arranges and carries out individual and group presentations to Fallon Health Weinberg staff, community groups, and agencies.
Maintains consistent contact with referral sources.
Researches community outreach activities appropriate to the Fallon Health Weinberg market.
Participates in community outreach activities, such as senior fairs.
Maintains referral tracking system and contact file for evaluation purposes.
Prepares monthly referral and enrollment/disenrollment reports and other pertinent statistics.
Meets regularly with the Fallon Health Weinberg Intake Coordinator to discuss progress of referrals and solve problems.
Assists the Intake Coordinator as needed and serves in that position during Coordinator’s time off.
Participates in the quality improvement activities related to the referral and enrollment processes as necessary
Build relationships with appropriate partners at local Senior Centers, Housing authorities, offices of the aging, home care companies, community organizations and other resources. Drop by regularly to ensure all marketing materials are available and promote referrals and lead generation.
Track and report on all sales activities.
Demonstrate strong sales skills and willingness to “own” a prospect from start to finish..
Handle all leads by mailing out Sales Kits, following up with an outbound calls in order to promote home visit appointments
Update referral information as necessary throughout the enrollment process
Work with referral to determine areas preventing enrollment and resolve.
Adhere to regulatory guidlelines regarding the Fallon Health Weinberg MTLC
Maintains a personal touch in all correspondance and communication with prospects.
Work very closely with team members to solve customer problem. Offers solutions and suggestions for process and product improvement to management.Determine, understand and meet prospect needs.
Assist in training activities as needed.
Other duties as assigned
Associates Degree or equivalent combination of education and experience.
Knowledge of customer service and marketing principles and practice
2 years’ experience in sales or service environment preferably “customer facing”
Previous experience coordinating sales activities and events as well as providing support in the field.
Strong knowledge of Microsoft Office products
Demonstrate strong customer service skills and willingness to “own” a prospect from start to finish
Excellent verbal and written communications skills.
Ability to work independently and exercise sound judgment.
Ability to establish effective relationships with community members, referral sources, and co-workers.
Ability to maintain accurate records and prepare clear, concise reports, correspondence, and other written materials.
Possession of a valid New York driver’s license and a vehicle to be used for marketing activities and home visits.
