Brief Summary of Position:

Responsible for conducting education and wellness coaching telephonically and person to person for members with chronic conditions to identify member’s knowledge, skills and behavioral needs, and gaps in care. The Health Educator provides individualized education and coaching designed to reinforce positive lifestyle changes that can lead to reduction in hospitalizations. The Health Educator should be able to demonstrate knowledge and application of: health promotion, behavior change and population based theories and models; and is familiar with health communication strategies.


Primary Job Responsibilities

  • Perform telephonic health coaching for members that provides comprehensive, culturally appropriate counseling and education on diabetes and heart disease and other conditions as appropriate

  • Educate at risk patients about lifestyle behaviors that affect risk

  • Implement and/or coordinate individually focused personal health plan to encourage member self-management

  • Respond to inbound calls from members to assist them in accomplishing personal health goals

  • Assist with increasing members motivational level by addressing barriers and addressing needs

  • Coordinate with the multidisciplinary team (Health Promotion/Quality Management, Case Management) regarding methods/approaches to help participants recognize and overcome barriers to improve their health

  • Manage provider office communications related to member health gaps

  • Provide face to face individual or small group sessions with members on assigned panel

  • Implement consistency in the use of Chronic Conditions Guidelines across the spectrum of care

  • Maintains up-to-date knowledge of project-related health education subjects and current certification/training as directed by manager

  • Be able to work with people of all ages, educational levels and multiple health issues

  • Strictly observes HIPPA regulations and the FCHP policies regarding confidentiality of enrollee/member information



Bachelor's degree in health related field


Certified Health Education Specialist (CHES) or Certified Diabetes Educator (CDE)

License: Active Driver’s license and reliable transportation

Certification : Certification in Case Management preferred


Other: Satisfactory Criminal Offender Record Information (CORI) results


  • Minimum 3 years’ experience conducting health education counseling with individuals and groups

  • Knowledge of chronic conditions including heart disease, diabetes, and familiarity with treatment and medication regimens for these conditions and other conditions as appropriate

  • Strong desire to enhance/change member’s/populations knowledge base on health-related issues

  • Prior telephonic health education coaching experience preferred

  • Knowledge and training in behavior change theory

  • Training in motivational interviewing techniques a plus

  • PC skills within windows based environment required

  • Experience with telephonic interviewing skills and working with a diverse population, that may also be Non-English speaking, preferred

  • Independent learning skills and success with various learning methodologies including but not limited to: self-study, mentoring, classroom, and group education

  • Working with an interdisciplinary care team as a partner demonstrating respect and value for all roles and is a positive contributor within job role scope and duties

  • Willingness to learn insurance regulatory and accreditation requirements

  • Knowledgeable about software systems including but not limited to Microsoft Office Products – Excel, Outlook, and Word

  • Familiar with Excel spreadsheets to manage work and exposure and familiarity with pivot tables

  • Accurate and timely data entry

  • Effective teaching skills and the ability to assess a member’s knowledge level and the ability to develop and implement a care plan that meets the member’s need

  • Knowledge about community resources specific to the health condition(s) being taught

  • Physician and other health care provider interaction and other communication

  • Ability to effectively respond and adapt to changing business needs and be an innovative and creative problem solver

  • Familiarity with Mass Health and Medicare criteria and levels of care

  • Familiarity with NCQA population health requirements
