About Summit ElderCare:
Fallon Health operates one of the largest Programs of All-Inclusive Care for the Elderly (PACE) in the country. Called Summit ElderCare, Fallon’s PACE program helps older adults and their caregivers to avoid or delay nursing home care. Participants in Summit ElderCare are cared for by a team of professionals and have access to social support and a wide array of medical services in one convenient location while they keep the independence of living in their own homes and communities.
Brief Summary of Purpose:
Successful implementation of the Quality Improvement Plan, including development of an annual work plan, conducting educational needs assessments, implementing training plan and maintaining supporting documentation in these areas.
Responsibilities
Primary Job Responsibilities:
Identify opportunities for program improvement through monitoring of incident and grievance reports
At the direction of the Quality and Utilization Management Committee and leadership develop plan, organize, and implement the SE Quality Improvement Program
Analyze data and generate reports to address quality of care and risk issues
Oversee the process of investigation and reporting of incidents and grievances.
Ensure maintenance data that supports clinical quality improvement activities, HPMS reporting, and other external reporting requirements. Analyzes clinical tracking logs and monitors for patterns and trends
Development and delivery of educational programs based on annual training needs assessment
Ensure program compliance with PACE personnel training requirements
Support site specific quality initiatives
Qualifications
Education:
Bachelor’s degree required.
License/Certifications:
RN Licensed in Massachusetts
Experience:
5 years’ experience working in health care setting, one year in related area of performance improvement and/or staff education
Excellent verbal and written communication skills; strong presentation skills
Supervisory or project management experience preferred
